PHS Transportation Information / Formulario para Solicitar Transporte

We are excited to have free bus service to all of our students to and from school. To better prepare and provide the best service, we are trying to get an accurate count of which of our students need transportation and where to locate our bus stops.

Please fill out the attached Transportation Request Form. 

Estamos emocionados de tener un servicio de autobús gratuito para todos nuestros estudiantes hacia y desde la escuela. Para prepararnos mejor y brindar el mejor servicio, nos gustaría obtener un recuento preciso de cuáles de nuestros estudiantes necesitan transporte y dónde ubicar nuestras paradas de autobús.
